RESUMO

Modulation of microglial response could be a target to reduce neuroinflammation associated with Alzheimer's disease. In this study, we propose that lipophilic bioactive molecules present in pomace olive oil (POO), transported in triglyceride-rich lipoproteins (TRLs), are able to modulate microglial high-oleic sunflower oil (HOSO, points) or pomace olive oil (POO, stripes). In order to prove this hypothesis, a randomized crossover postprandial trial was performed in 18 healthy young women. POO was assayed in opposition to high-oleic sunflower oil (HOSO), a common dietary oil which shares with POO an almost identical fatty acid composition but lacks certain biomolecules with recognized antioxidant and anti-inflammatory activities. TRLs were isolated from blood at the baseline and 2 and 4 hours postprandially and used to treat BV-2 cells to assess their ability to modulate the microglial function. We found that the intake of POO leads to the constitution of postprandial TRLs that are able to modulate the inflammatory response in microglia compared to HOSO. TRL-derived POO reduced the release of pro-inflammatory cytokines (tumor necrosis factor-α, and interleukins 1ß and 6) and nitric oxide and downregulated genes codifying for these cytokines and inducible nitric oxide synthase (iNOS) in BV-2 cells. Moreover, the ingestion of POO by healthy women slightly improved glycemic control and TRL clearance throughout the postprandial phase compared to HOSO. In conclusion, we demonstrated that consuming POO results in postprandial TRLs containing lipophilic bioactive compounds capable of regulating the inflammatory response prompted by microglial activation.

BACKGROUND: Interprofessional collaborative practice (ICP) is currently recommended for the delivery of high-quality integrated care for older people. Frailty prevention and management are key elements to be tackled on a multi-professional level. AIM: This study aims to develop a consensus-based European multi-professional capability framework for frailty prevention and management. METHODS: Using a modified Delphi technique, a consensus-based framework of knowledge, skills and attitudes for all professions involved in the care pathway of older people was developed within two consultation rounds. The template for the process was derived from competency frameworks collected in a comprehensive approach from EU-funded projects of the European Commission (EC) supported best practice models for health workforce development. RESULTS: The agreed framework consists of 25 items structured in 4 domains of capabilities. Content covers the understanding about frailty, skills for screening and assessment as well as management procedures for every profession involved. The majority of items focused on interprofessional collaboration, communication and person-centred care planning. DISCUSSION: This framework facilitates clarification of professionals' roles and standardizes procedures for cross-sectional care processes. Despite a lack of evidence for educational interventions, health workforce development remains an important aspect of quality assurance in health care systems. CONCLUSIONS: The multi-professional capability framework for frailty prevention and management incorporated interprofessional collaborative practice, consistent with current recommendations by the World Health Organization, Science Advice for Policy by European Academies and the European Commission.

RESUMO

Se presenta una experiencia práctica de implantación de la Cuádruple Hélice basada en el Modelo de Innovación de los Reference Sites de la Comisión Europea, a través del método Design Thinking. Este método establece que, para alcanzar con éxito el diseño de una solución digital, deben cubrirse cinco fases ineludibles: empatizar, definir, idear, prototipar y evaluar. Se utilizó un diseño de investigación-acción y se aplicaron técnicas cualitativas de recogida y análisis de información en cada fase: metasíntesis cualitativa, grupos focales, entrevistas biográficas, foros profesionales y comunitarios, técnicas de prototipado y testeo con personas usuarias. La aplicación sistemática de este método ha facilitado la consolidación de un modelo de trabajo cooperativo, en el que interactúan los principales agentes interesados en el envejecimiento activo y saludable. Se han creado así productos y servicios que satisfacen mejor las necesidades de las personas usuarias, haciéndolas parte activa del proceso de creación

Introducción: En la última década son numerosos los hospitales que han iniciado su actividad en pacientes candidatos a un implante coclear (IC), y se han producido numerosos y relevantes avances para el tratamiento de la hipoacusia neurosensorial que han desembocado en una ampliación de las indicaciones de los IC. Objetivos: Ofrecer a los especialistas de otorrinolaringología, de otras especialidades médicas, autoridades sanitarias y a la sociedad en general una guía clínica sobre implantes cocleares. Métodos: Las comisiones científicas de otología, otoneurología y audiología de la Sociedad Española de Otorrinolaringología y Cirugía de Cabeza y Cuello (SEORL-CCC), de manera coordinada y consensuada, han llevado a cabo una revisión del estado actual de los IC basándose en las reglamentaciones existentes y en las publicaciones científicas que se referencian en la bibliografía del documento elaborado. Resultados: La guía clínica sobre implantes cocleares aporta información sobre: a) definición y descripción sobre IC; b) indicaciones de los IC; y c) requisitos organizativos para un programa de IC. Conclusiones: Se ha elaborado por un comité de expertos de la SEORL-CCC una Guía clínica sobre implantes cocleares que aporta coordenadas de actuación para todos aquellos agentes de la sanidad en la toma de decisiones en el ámbito de los IC como forma de tratamiento de la discapacidad auditiva

Objectives: To provide a guideline on cochlear implants to specialists in otorhinolaryngology, other medical specialities, health authorities and society in general. Methods: The Scientific Committees of Otology, Otoneurology and Audiology from the Spanish Society of Otolaryngology and Head and Neck Surgery (SEORL-CCC), in a coordinated and agreed way, performed a review of the current state of CI based on the existing regulations and in the scientific publications referenced in the bibliography of the document drafted. Results: The clinical guideline on cochlear implants provides information on: a) Definition and description of Cochlear Implant; b) Indications for cochlear implants; c) Organizational requirements for a cochlear implant programme. Conclusions: A clinical guideline on cochlear implants has been developed by a Committee of Experts of the SEORL-CCC, to help and guide all the health professionals involved in this field of CI in decision-making to treathearing impairment

This paper shows an experience of the practical implementation of the Quadruple Helix-Based Innovation Model of Reference Sites of the European Commission through the Design Thinking method. This method establishes that, in order to successfully achieve the design of a digital solution, five unavoidable phases must be covered: empathize, define, ideate, prototype and test. The process was approached using the research action design. Qualitative data collection and analysis techniques were applied in each phase: qualitative meta-synthesis, focus groups, biographical interviews, professional and community forums and techniques of prototyping and testing with users. BACKGROUND AND OBJECTIVE: Although elder abuse is not a new phenomenon, it remains hidden. There have been carried out various preliminary studies about the prevalence of elder abuse in different countries. The aim of this study is to estimate the prevalence of suspicion of elder abuse in old persons without cognitive impairment, dwelling in community, who were attended in Primary Health Care or Social Services Centres. PATIENTS AND METHOD: We carried out a transverse study in which 340 elders participated. RESULTS: We found a 12.1% prevalence of suspicion of elder abuse. Psychological abuse suspicion was the most frequent type and it was very common the simultaneous presence of different types of abuse (psychological and physical and sexual). The suspicion of elder abuse was more frequent in women and spouses were responsible in a high great frequency. CONCLUSIONS: The information obtained allows advancing in the knowledge of elder abuse in Spain, where the research about this issue is poor. However, the prevalence found neither has to be considered as a social alarm nor as a social slackness.

OBJECTIVE: To compare the Robert Jones bandage with the "U" splint in edema control of ankle fractures. MATERIAL AND METHODS: A prospective, longitudinal study, open, comparative and randomized made in 32 male patients (72%) and 16 female patients (28%), the average age was 36.9 years. Patients with an ankle fracture within 12 hours of evolution were registered. Both ankles previous to immobilization were measured, dividing the patients in 2 groups. Group 1 were treated with Robert Jones Bandage, and group 2 with "U" splint. The pain previous to immobilization was registered using an analogous visual scale. Twenty-four hours later the circumference and the pain was measured again. RESULTS: Group 1 displayed less edema, mean 17.69 mm, in comparison with group 2, of 8.33 mm (p < or = 0.36). Pain previous to immobilization in group 1 was a mean of 7 and in group 2 of 5.5 (p < or = 0.3); 24 hours later, a mean of 2 in both (p < or = 0.8). CONCLUSIONS: There were no significant differences between both immobilization methods.
